Abstract

Pregnant women are more susceptible to respiratory pathogens and severe pneumonias because of pregnancy changes. The objective of this article is to describe the anesthetic conduction of a pregnant woman with a pre-term pregnancy and respiratory failure because of COVID-19. The patient with 33.3 weeks of gestation and without positive personal history needed an emergent Caesarean section. She was admitted to the Intensive Care Unit because of respiratory failure secondary to bilateral pneumonia caused by COVID-19. The patient needed a non-invasive respiratory support. She was announced for urgent anterior segment Caesarean, under neuro-axial intra-thecal anesthetics with hyperbaric bupivacaine and supplementary oxygen by Venturi mask to 10 l/min that keeps stable the cardiac respiratory parameters. The newborn gets Apgar 7/9 and develops satisfactorily, as well as his mother, until they were discharged from the respective Intensive care Units. The adequate pre-operatory evaluation, selection and anesthetic conduction, as well as, to state a multidisciplinary strategy, constitute the pillars for avoiding fatal complications.
